    Registered Dietitian
    Permanent Part-Time (0.8 FTE)
     
      
     
    Working as a member of the Team-Based Care Program, the Registered Dietitian provides comprehensive nutrition services to improve the health of PCN patients. This position is primarily located in the Leduc central PCN office with the possibility of working within the clinics of member physicians.  The role will support the mission of providing quality, collaborative team-based care providing education, resources and support for the patient’s health journey.
     
    Key Responsibilities:
    • Conduct nutrition assessments, initiate and implement nutrition care plans and follow-up for patients referred by member physicians and other health providers;
    • Coach and support behavior change across the lifecycle;
    • Employ health promotion and health education strategies to support health behaviour change;
    • Act as a nutrition resource to members of the multidisciplinary team and PCN physicians;
    • Develop and deliver group education programs;
    • Coordinate services and care with patients including facilitating referrals to appropriate service providers and community supports.
     
    Hours of work will be dependent upon location but generally scheduled between 8 a.m. to 9 p.m. Monday to Friday.
     
    Qualifications:
    • Bachelor’s degree with a major in Foods and nutrition;
    • Current registration with the College of Dietitians of Alberta;
    • Previous experience could include working in a primary care setting, group facilitation or instruction, using health behaviour change strategies e.g. motivational interviewing;
    • Ability to make professionally autonomous decisions in coordinating patient care;
    • Experience working with multi-disciplinary teams;
    • Strong effective written and oral communication skills;
    • Current criminal record check;
    • Must have own vehicle and valid driver’s license.
